WIP: perfect foresight controlled paths 

TODO:
– support stack_solve_algo=7 (reusing function for stacked Jacobian substitution)
– support linear option
– deprecation warning for det_cond_forecast/init_plan/flip_plan
– reference manual:
  + document the restrictions over block/bytecode/stack_solve_algo
  + remove det_cond_forecast?
