Cholesky decomposition: only add to diagonal if really necessary

Closes #1891 (closed). Incidentally harmonizes the IRF computation criteria between stoch_simul and PosteriorIRF_core1 (the latter conditioned on the diagonal of the Cholesky factor instead of the variance).
